The Knox County Health Department’s CHANT (Community Health Access and Navigation in Tennessee) program helps link participants to health services and community resources to make sure they have the care they need.
It’s FREE for pregnant women and any adult with a child in the home under the age of 21. CHANT can also assist families with cost of copays, medical treatments and supplies for children who have a qualifying medical diagnosis.
CHANT Care Coordination Services: Care Coordinators provide quality support and will link you to available resources specific to your family's individual needs. This includes assistance with social services, finding a medical provider, tracking your baby's development and providing child health and development information based on your child’s age.
Children’s Special Services: CSS provides reimbursement for comprehensive medical services for children with physical disabilities from birth to 21 years of age. CSS eligibility depends on income and qualifying diagnosis.
